Everyone hates paperwork! Including us!
We really prefer to spend our time at the examination visit getting to know you and doing all we can to answer your questions about orthodontic care. The forms on this page are necessary in order to answer your questions and to perform the care as safely and effectively as possible. Putting them on this website allows you to print them at home and complete them at your leisure rather than rush through them at the exam visit. You may either mail them to us ahead of time or bring them with you at your first visit. Of course it is not required that you complete these ahead of time but the answers to some of your questions may depend on the information.
The New Patient Registration Form on this page is a good way to get started in our office and is the only form that may be submitted on-line. You may complete it in the office if you don’t wish to do so on-line. The Medical and Dental History Forms are essential for safe care and good communication with your dentist. They can be printed from this page and completed if you wish. This saves time at the examination visit.

Bring the completed form with you to the examination visit or send it in ahead of time to get the jump on having the best information.
